MSA Summer Clinic Registration Form

The cost of the Summer Clinic is $99. When you send in your registration, you will be sent an e-mail confirmation along with a medical release form which should be mailed in with you payment. You may also bring in your registration or payment to MSA. Please call (734) 944-6721 if you have any questions. You will be redirected to michigansportsacademy.com after you submit form.
